Short course high dose steroid Options

After a recent flare, my doctor prescribed a five day course of Prednisolone (30mg) as Naproxen and increased dose of MTX (17.5mg) have so far not reduced my pain and discomfort.

Although the Prednisolone is for a very short time, the dose is quite high and I am nervous about taking it. At the same time realise it would be the most sensible thing to do, as it would probably reduce inflammation dramatically. I also am aware that the nasty side effects associated with steroids are usually with long term use.

Has anyone had a similar experience of a high dose over a short period?

I would be very grateful for any advice.

Hi Eleanor

A short dose such as you describe is unlikely to cause you any side effects since most of them are associated with long term steroid usage.

Also 30 mg is a moderate dose. I have had doses up to 80mg in the past and they did cause some side effects.

Even then, balancing the side effects against the good effects, the medication was worth taking. However we all differ and you need to comfortable with it.

Hi. I regularly have high dose short courses and my GP is so confident in my ability to be sensible I only have to go and ask when I need them (perhaps once or twice a year).

I usually have 20mg per day for a maximum of two weeks. The longest and highest I had was 50mg which I then had to taper down over a period of 3 months. The only side effects I had were pounding heart and restlessness at night.

If you're taking steroids, particularly doses of over 20mg, for longer than 2 weeks you should be prescribed calcium alongside them as long term use can cause oesteoporosis. Calcium over the counter is no good, this calcium is different.

Good luck. Steroids are my magic wand when I have a flareup!

Me too but my rheumy consultant said something strange about them when I mentioned that I had been put on a 60mg course by A&E. 'It's not a dose rheumatology would use.' So I asked what rheumatology would use and was told 7.5 mg as a max! That's interesting because I have had several attempts at getting below 20 mg and have never succeeded yet. I have tried going down in 5 and 2.5mg steps. I am now trying 1 mg steps. I am down to 18 and the top of my pelvis is a slow burning fire, if you know what I mean.

Oh and yes - the heart palpitations are disturbing aren't they, especially noticeable at night and when lying on your side? I also had terrible nightmares, although that could partly be due to the amitryptiline.

I think they say 7.5 as a max dose as thats about what your body would produce naturally. I rarely get it that low andhave stopped worrying about it now. Currently working on 9mg one day and 10 the next last week, this week 9mg two days 10mg one day, and like you am already struggling with knees again, so shall probbly keep it at that for the next couple of week
